When to use this form

Use this form if you wish to start paying Additional Voluntary Contributions (AVCs) to increase your pension benefits.

Before you start

You will need:

your National Insurance number
your employer and payroll details
the amount you wish to contribute

You may wish to review AVC information first.

The form usually takes around 10 minutes to complete.
