When to use this form

Use this form to tell us if your personal details have changed, such as your address, name, marital status, email address, or telephone number.

Keeping your details up to date helps ensure your pension record is accurate and that you receive important information about your pension.

Before you start

You will need:

  • your National Insurance number

  • your current contact details

  • details of the changes you want to make

  • supporting documents if your name or marital status has changed

The form usually takes around 5 minutes to complete.
