Nominate who receives your death grant

When to use this form

Use this form to tell us who you would like to receive any death grant payable if you die. The pension fund will consider your wishes when deciding who should receive the payment.

Before you start

You will need:

your National Insurance number
names and contact details of your nominated beneficiaries
the percentage you wish each person to receive

You can nominate more than one person.

The form usually takes around 5–10 minutes to complete.
