When to use this form

Use this form to nominate your cohabiting partner for pension benefits in the event of your death. This helps the fund understand your wishes when making benefit decisions.

Before you start

You will need:

  • your National Insurance number

  • your partner’s personal details

  • details about your relationship and living arrangements

The form usually takes around 10 minutes to complete.
