Notify us of a member’s death
When to use this form
Use this form to notify the Cheshire Pension Fund that a member has died. This allows us to begin processing any benefits due and provide support to beneficiaries.
Before you start
You will need:
the member’s full name
date of birth and National Insurance number (if known)
date of death
your contact details
You may be asked to provide a copy of the death certificate.
The form usually takes around 5 minutes to complete.
